Urban Peaks Rehab is happy to accept patients enrolled in Colorado Medicaid insurance.

In order to qualify for Medicaid your income must fall at or below 133 percent of the federal poverty level.

In order to receive coverage for methadone for the treatment of opioid use disorder, federal law mandates that patients are enrolled in, or have documented proof of, substance use disorder counseling.

Medicaid  provides healthcare options for families and individuals with limited resources. Medicaid is the largest source of funding for medical and health-related services for people with low income in the United States.

Urban Peaks Rehab is a contracted in network provider for several commercial insurance programs. Your individual benefits will vary based on your plan. You may contact your health to determine what your exact benefits are prior to your appointment. Urban Peaks will also verify benefits the day of your appointment to determine copayments and requirements for prior authorization if any exist.

If your health plan is not listed, you may have out of network benefits that can cover a portion of your costs at Urban Peaks Rehab. We also offer affordable self-pay rates. These rates are often cheaper than going through insurance so contact the office to inquire.
